South Africa's health system is often described as two separate arenas: public and private. But in practice, patients move between the two more than we tend to acknowledge, and increasingly, so do their doctors. At Insight's 2025 Foresight event, Co-Create the Future, Actuarial Consultant Shannon Phillips shared new data on exactly this "missing middle": the millions of South Africans who use private primary care while relying on the public sector for specialist and hospital-level treatment.

Her presentation put numbers to something Vula sees every day, that referrals, advice and follow-up are already flowing across the public-private divide, and that connecting the two sectors properly changes outcomes for patients caught in between.
